HOW MEMBERSHIP WORKS
A Flat Monthly Fee. Direct Access to Your Provider.
At Rural Authentic Wellness, you pay a flat monthly fee directly to the clinic rather than having it billed through insurance. In exchange: unlimited primary and urgent care visits, same-day or next-day scheduling, and direct messaging with your provider through Elation Patient Passport and Spruce. No phone tree, no hold music.
Most visits are covered by membership. Labs, aesthetics, and weight-loss programs are billed separately at direct cost, with no insurance markup.

TRANSPARENT rATES
No Surprise Bills
No facility fees and transparent pricing. Non-members can access DOT physicals, sports physicals, and acne consultations at flat rates, no membership needed.
Lab work is billed at direct cost with no insurance markup. Members receive additional discounts on top of already-direct pricing. You know the cost before the draw.
HSA-Eligible: As of January 2026, DPC memberships are HSA-eligible (IRS Publication 969). If you're on a high-deductible plan, pre-tax HSA dollars can offset your monthly membership fee.

INSURANCE VS. HEALTHSHARE
Membership Works With or Without Your Coverage
You don't have to cancel your insurance to join. DPC is a membership-based model; you pay the clinic directly, separate from any insurer.
The most common setup is pairing membership with a HealthShare plan. HealthShare programs, like Zion Health, are cost-sharing arrangements, not traditional insurance, that cover major medical events. DPC handles the everyday care. HealthShare handles the big stuff.
A typical Minnesota family on traditional insurance might pay $1,400–$1,800/month in premiums with a $6,000–$10,000 deductible before coverage kicks in. Under DPC + HealthShare, the same family often pays the $240/month family max plus a lower HealthShare contribution and gets better access.
HORMONE HEALTH
Hormone Care for Women and Men at Rural Authentic Wellness
Rural Authentic Wellness sees both women and men for hormone health. If you've been told your labs look fine but still feel off: fatigue, weight changes, brain fog, low drive, disrupted sleep, a full hormone workup is the right starting point, not a dismissal.

Perimenopause, menopause, hormonal weight gain, brain fog, disrupted sleep, and cycle irregularities. Bioidentical hormone therapy (BHRT) is fully explained before any prescription.

Men's Hormone Health
Low testosterone is consistently underdiagnosed in primary care. If your energy, strength, or focus has declined, a proper workup starts here, not a referral that takes months.
Membership Required: Men's HRT requires a Rural Authentic Wellness clinic membership. Initial HRT appointment is $165 for members. All follow-ups are included. Men's HRT requires closer monitoring and more frequent appointments.

Telehealth Available Statewide
Hormone appointments, including the initial appointment, are available by telehealth across Minnesota as long as required labs can be completed. We will order and send lab requisitions to your preferred location so you can get drawn locally.
A note on labs:
If labs are not completed at our Olivia clinic, Rural Authentic Wellness is not responsible for the cost of labs drawn at an outside facility. We can order and send your requisition, but the drawing facility sets its own pricing.
Supervised Weight Management Program
Clinically supervised programs are built around your metabolic baseline, health history, and how your body has responded to past efforts. GLP-1 medications are available where clinically appropriate, as one tool in a supervised plan, not a standalone prescription.

On GLP-1 Medications: At Rural Authentic Wellness, GLP-1 therapy is paired with ongoing medical guidance, regular follow-up, and personalized support to help you achieve safe, sustainable results. Long-term maintenance and transition planning are built into every program.
Members and Non-Members: Available as a standalone program for non-members. Members benefit from having weight management and primary care managed by the same provider.
Certified DOT Physicals
FMCSA-certified DOT physicals for commercial drivers, same-day medical certificate, same-week scheduling. No long drives, no multi-week waits.
Required for CDL holders operating vehicles over 10,001 lbs GVWR across state lines, transporting passengers for compensation, or hauling placarded hazardous materials. Drivers managing controlled conditions (hypertension, diabetes) are not automatically disqualified.

Sports Physicals
Pre-participation sports physicals meet all Minnesota school requirements and are completed in full at the visit—forms signed and ready the same day. Same-week and last-minute scheduling is available for student athletes.

Acne Consultations
If OTC products haven't worked, the issue is usually the root cause, not the product. The consultation evaluates your skin type, hormonal picture, triggers, and health history. A treatment protocol is built from that assessment.
Hormonal acne, tied to your cycle, perimenopause, or contraceptive changes, is a specific focus here. It's frequently undertreated in standard primary care, and a proper clinical evaluation is the right starting point.
